The purpose of this position is to ensure a welfare service of the highest quality is provided to clients, families and other relevant stakeholders of the Association; and be the main point of contact for liaison with government and non-government agencies in relation to welfare services for the Association’s clients and families.
3. Primary Duties and Responsibilities
(a) Maintain, and develop where possible, current welfare services provided by the Association for clients and families affected by Huntington’s Disease
(b) In conjunction with the Senior Welfare Officer undertake initial assessment of all new client referrals to the Association and access welfare services where appropriate
(c) Organise and implement quality management issues relating to client and family care such as individual care plans
(d) In conjunction with the Senior Welfare Officer determine clients’ suitability to access day respite and other services provided by the Association
(e) In conjunction with the Senior Welfare Officer coordinate and implement client/family support and recreation groups
(f) Maintain accurate and current resources for dissemination to clients, families and care providers
(g) Coordinate the provision of education and training to external stakeholders as required
(h) Coordinate networking of relevant health and disability professionals.
(i) Prepare and present reports to the Management Committee as required.
